The purpose of this project is to identify, develop, and evaluate the
components and elements of flexible funding that enable and support consumers
to receive coordinated services from multiple state departments. The goals
of this research project are to identify how funding impacts the development
and implementation of transition plans for 17- to 20-year-old youth with
developmental disabilities, and to design a flexible fiscal roadmap for
assuring the integrated delivery of state and community services that
support transition planning. CMS has provided funding for this 3-year
project under the 2001 Real Choices Systems Change grant solicitation.
