Person-Centered Services Performance Measures

The purpose of this project is to fund the development of cross-system performance measures to measure the State’s success at achieving community integration for persons with disabilities. CMS has provided funding for this 3-year project under the 2003 Real Choices Systems Change grant solicitation.
